Health Foundation of La Porte (HFL) is dedicated to our mission of empowering La Porte County residents to live healthy and well.
Health and wellness are at the core of HFL’s work. For HFL:

Health is “a state of complete physical, mental and social wellbeing and not merely the absence of disease or infirmity” (World Health Organization).

Wellness is the “active process through which people become aware of, and make choices toward, a more successful existence.” Wellness has six dimensions: emotional, occupational, physical, social, intellectual, and spiritual (National Wellness Institute).
HFL makes grants to advance our strategic priorities of Healthy Children, Healthy Living, and Healthy Minds. We make Healthy La Porte grants to respond to community needs outside of our strategic priorities that demonstrate a connection to our mission and vision. HFL also funds efforts to strengthen nonprofit organizations in La Porte County through Healthy Partners grants, and we work to ensure our community has a safe, manageable, and efficient program to help community members survive cardiac arrest through our When Seconds Count – AED Initiative. Additionally, HFL makes grants to respond to emergencies that affect the health and wellness of La Porte County residents, such as the case with HFL’s COVID-19 Emergency Grants.
Many issues affect the health and wellness of La Porte County residents. HFL is committed to supporting a diverse range of health and wellness projects, programs, and services to achieve the vision of La Porte County becoming one of the top 10 healthiest communities in Indiana. We envision partnering with many types of community organizations through strategic, thoughtful grantmaking to invest in becoming a healthier community together.
